This is the "Tutorial case" from Tutorial 6 (Creating Web Page Forms) This example illustrates:
  1. A simple fixed-width layout. The two-row heading (in a thead row group) uses a rowspan=2 for the first row, and the remaining rows use colspan=2.)
  2. Input boxes, radio buttons, checkboxes, a selection box, a textarea field, a hidden field, and a submit button, a reset button (called Cancel),
  3. Two fieldsets (enclosing the radio buttons and the checkboxes), and
  4. Label tags on all field labels

The "enhancements" relative to the textbook are:

  1. The onload widget that displays this help text.
  2. The layout table has been simplified. In the textbook, several rows of the page layout table are used for the various sections of the form. In this example, the entire form is in one layout table cell.
  3. The form tags are more appropriately located close to the form itself. That is, in the book, since the form is split among several layout table rows, the form tags must be at the beginning and end of the page, but in this example, the form tags are within the table cell that contains the form.
  4. This example uses <tbody>, <tfoot>, and <thead>; the book only uses <thead>
  5. A padding-left style provides some horizontal whitespace between the radio buttons and the checkboxes.
